Medical Consent & Authorization: I give permission for the child named above to attend 5 DAY CLUB - THE GOOD NEWS OF JESUS. In the unlikely event of an emergency, I give my permission for the child named above to be treated by an accredited physician in an approved emergency clinic or hospital. I, therefore, designate the 5 DAY CLUB leaders and staff to act on my behalf for appropriate treatment. I further release Mt. Zion Lutheran Church and its officers and leadership from any liability in the event of an accident during the daily activities of 5 DAY CLUB - THE GOOD NEWS OF JESUS. Information received is confidential and is being gathered for the purpose of caring for your child while attending the 5 DAY CLUB at Mt. Zion Lutheran Church. Any medical information collected allows Mt. Zion Lutheran Church staff and volunteers to obtain medical assistance in emergencies. If your child is ill or injured during the 5 DAY CLUB, we will contact parents/guardian, listed immediately and then any other emergency contact. I also hereby give permission to Mt. Zion Lutheran to use photographs, slides or videos of myself or dependent(s) for purposes of the 5 DAY CLUB-related activities and promotions. *
