Medical Consent & Authorization: I give permission for the child named above to attend TRUE NORTH-Trusting Jesus in a Wild World 2025. In the unlikely event of an emergency, I give my permission for the child named above to be treated by an accredited physician in an approved emergency clinic or hospital. I, therefore, designate the VBS leaders and staff to act on my behalf for appropriate treatment. I also give my permission for an authorized VBS personnel (nurse) to administer medications to my child as deemed medically necessary. I further release Mt. Zion Lutheran Church and its officers and leadership from any liability in the event of an accident during the daily activities of TRUE NORTH-Trusting Jesus in a Wild World 2025. Information received is confidential and is being gathered for the purpose of caring for your child while attending VBS at Mt. Zion Lutheran Church. Any medical information collected allows Mt. Zion Lutheran Church staff and volunteers to obtain medical assistance in emergencies. If your child is ill or injured during VBS, we will contact parents/guardian, listed immediately and then any other emergency contact. I also hereby give permission to Mt. Zion Lutheran to use photographs, slides or videos of myself or dependent(s) for purposes of VBS-related activities and promotions. *
